Well, I'm from Croatia. And one of our magazines had an interview with Mira Furlan. She said that she asked producers to kill her off, because she had enough of Hawaii (''island fever''). But she also said she might be back for Season 5 (!)

She got bored of Hawaii. She got ''island fever'' - that's how cast of Lost calls it in joke. Simply, she didn't want to be on that island, she didn't want to work that job, so she made a deal with the producers to kill her character off.

Shot her. But, her character is pretty popular with fans of the series, so they leaved possibility for comeback next season.

Same here. By killing her off, the chances of seeing her back story are very small. Besides I think if they want to put it in the serie, it should be way earlier, for example in season 2.
