My impression (since this happened either right after or right before one of the flashbacks) was that Jack was thinking about his father and he confused Ethan with his father... he wouldn't want his father to let another patient die, hence "not again"; likewise, he wouldn't want anyone to die because of Ethan ecause he (Jack) hadn't stepped in and spoken up about it.

That's what I like so much about "Lost" in the early going. It conceptualized as much as a TV drama can, how it might really be like for folks who suddenly find themselves totally out of their element and having to live by their wits. It only makes sense they would reconstruct life on the island based on their previous experiences. Up til this point everything is making sense because their actions seem to have logical explanations based on who they were.

We will find out that for many of them, their previous experiences have their origin on crazy island.

Yeah, all the buzz seems to point to that but I still think they could have Dominic Monaghan on another ABC show and he'd still show up for the final season. From what I've read, this will be the case with Elizabeth Mitchell during the first season of V. As long as they bring him back in a believable way (well, a believable way for Lost, that is), I'll be elated. If it's in a stupid, cheap way, I think I will prefer if he had just remained dead.
